With Antonina Health Care, the personal touch has returned. House calls are back, and used for more than simple home health care!
We assist you in completing all of the required paperwork for your friend, neighbor, family member, or other loved one to get on the program that is most appropriate for their needs. Paperwork needed includes, but is not limited to Colorado Medicaid Application, referral to services for case management agency, required documentation from the physician.
Our nurses will follow up with clients after discharging from the hospital, rehabilitation or skilled nursing facilities (SNF) and provide information and recommendations for appropriate after care services. Information and recommendations may include DME, incontinence supplies, Adult Day Programs, transportation and coordination with assigned case manager.
